The form — 7 questions
Age → sex → smoking → diabetes → tooth (FDI, e.g. 36) → implant system → surgeon experience (or “Skip”).
X-ray (optional)
JPEG/PNG/DICOM. Metadata is stripped BEFORE storage. No patient name burned into the image.
Reply: code + AI prediction
The bot returns the case code (study_uuid) and a risk percentile. Reference only — the clinician decides.

At 6/12 months
The bot reminds you: one tap — success / periimplantitis / failure, optionally MBL (mm). Done.
What to tell the patient (live mode)
“Our clinic participates in an open scientific registry of implant outcomes. Data are de-identified — no name or phone. You may decline or withdraw consent at any time.” Have the consent form signed; the case code stays with the patient.
